Affidavit of Domicile Form
What is an Affidavit of Domicile? An Affidavit of Domicile form is used when a broker or financial institution requires proof of the place of residence of the deceased person (the “decedent”). The form is a sworn statement by the personal representative of the decedent concerning the name and place of residence of the decedent, as well as the place where the decedent died.
The information regarding the decedent’s place of death and place of residence can usually be found on the decedent’s death certificate. The form should be signed before a Notary Public by the personal representative of the decedent, either the Executor of their estate or the Successor Trustee of their trust.
