Durable General Power of Attorney
A durable general POA form is a very useful estate planning tool if you are concerned about who would assume authority of your finances and assets in the event you are unable to take care of those matters yourself. Unless you specifically include a “durability” clause in a general power of attorney, the general POA form terminates if the principal becomes mentally or physical incapacitated.

A durable medical power of attorney form also known as a healthcare power of attorney is used for allowing another person to make medical and health care decisions for you when you are no longer capable of making them for yourself as determined by a licensed doctor. In addition, a medical poa
lets you give specific instructions on how you want to be treated medically should this situation occur.
If designated by you in your health care power of attorney, your agent will be able to make all health-care decisions including the type of treatment, location of treatment, and in addition, the right to refuse or decline life prolonging treatment to keep you alive. Any special instructions can be included in this form so your wishes are followed exactly as you would like them to be.
When you choose someone to be your health care agent they are obliged to follow your specific instructions as given in your durable power of attorney for health care or health care proxy and advanced directive.

You may choose any individual who is 18 years or older. You can also choose another individual to be an alternate should your first choice as an agent is unable to full fill their obligation.

Financial Power of Attorney
A durable financial power of attorney form is a legal document which authorizes one person (Agent) to act for the individual granting the financial poa (Grantor) to make financial decisions on their behalf. If so appointed the person who is granted the power of attorney (Agent), will be able to act on the Grantor’s behalf for as few as one, or all financial transactions.
The Grantor may choose to have their financial power of attorney form become effective immediately or if they ever become incapacitated and cannot make decisions on their own. If no financial POA is prepared, and the individual becomes incapacitated, the court will appoint a guardian or conservator to make those decisions on their behalf.

Limited Power of Attorney
A limited power of attorney form is also referred to as a special power of attorney. This type of POA gives your agent the power to act on your behalf for a very limited purpose. This is a much better option for you if you do not want to give your agent broad, unrestricted powers (i.e. general power of attorney) but you need to designate someone to act on your behalf for one specific event.
